Renewal Process for Physician Assistants
This year physician assistants have a choice when renewing their licenses. On-line renewals are now available with the ability to complete the form and pay online using a credit card. A paper renewal form is also available on-line.
Physician assistants renewing on-line must complete the on-line renewal application form, including submission of the appropriate fee, no later than 11:59 pm Mountain Time, December 31, 2011. Payment may only be made by Visa or Master Card.
Physician assistants choosing to use a paper renewal form (found below) must submit the completed paper renewal application form and the renewal fee of $125 (which includes the $25 paper form processing fee), which must be received in the Board office no later than 5:00 p.m. on December 31, 2011. Faxes are not accepted. Payment may only be made by check or money order.
Licensees missing the December 31st renewal deadline who provide to the Board, in writing and not later than sixty (60) days after the renewal deadline, a statement of good cause for failure to submit their application for renewal in a timely manner, may be granted an extension of not greater than thirty (30) days by the Executive Director in which to submit their application, the appropriate renewal fee, and the license renewal grace period surcharge ($100).
A licensee whose request for an extension of the renewal application deadline is received by the Board more than sixty (60) days after the renewal deadline may be granted an extension for submitting their renewal application (and the required renewal fee and grace period surcharge) of not greater than thirty (30) days only with the approval of a majority of the Board officers.
Nothing herein imposes a duty on the Board of Medicine or the Executive Director to grant an extension to any licensee.
Licensees who fail to renew their license by the December 31st renewal deadline, and who are not granted a license renewal grace period, are required to go through the reactivation process to restore their license to active status.

Physician Renewal Process
FOR THE 2011-12 PHYSICIAN LICENSE RENEWAL ONLY, THE DEADLINE HAS BEEN EXTENDED TO AUGUST 31, 2011. This extra time is being used to implement an on-line license renewal system starting on June 1, 2011.
